Notes: 24/06/2018 Crisis Counselling: PRESENTING ISSUE: Kyle called Phoenix House as he was very upset and wanted support to help him cope with the grief of losing his close friend Jeremy. He initially spoke with receptionist Sara S who transferred him through to me. Kyle was then transferred to me as he was so emotional. BACKGROUND: Kyle is a 20 yo truck driver who lives in Bundaberg on his own. Within the last few weeks, his close friend Jeremy was killed in a motor car accident, and he has become so upset that he is unable to perform at work. He had informed his manager of the incident and is currently doing other duties temporarily until he is feeling better. Psychologically, Kyle was crying uncontrollably and found it difficult to articulate his feelings. He described never having felt like this before, and not knowing what to do. When asked, he did not have any thoughts of self-harm and he had never received any counselling in the past. Socially, Kyle is single and employed as a truck driver. He lives alone, but someone is moving in with him at the end of this week. ASSESSMENT: Kyle is suffering from an acute grief response secondary to the death of a close friend. He is not at risk of self-harm. As he requires grief/bereavement counselling, he does not fit the criteria for counselling from PH. INTERVENTIONS: (1) Counselling support to normalize the situation and affirm the pain of loss. (2) Safety plan made to call Lifeline is he does feel suicidal and to pull over if he becomes too distressed to safely manage his vehicle. (3) Referral to Centrecare provided for ongoing grief support after I called UC Bundaberg to get more information.
